    Health Risk Assessment from American Health Care

    The American Health Care Health Risk Assessment (HRA) is an on-line health questionnaire.  It is private, secure, and easy-to-use.

    You will be asked to enter basic health-related information only.  Once completed, you can view or print a personalized report called a Health Status Profile.  The Health Status Profile identifies your health risks and ways to promote a healthier lifestyle.  The HRA section called “Know Your Numbers” gives you important information to discuss with your doctor.  National health guidelines are associated with each section.  The evaluation provides a way for you to monitor your health in areas such as:

    • Chronic Asthma
    • Chronic Obstructive Pulmonary Disease (COPD)
    • Diabetes
    • High Blood Pressure
    • Migraines
    • Heart Disease
    • Major Depression
    • Stroke
    • Overweight/Obese Status
    • Osteoporosis
    • Smoking or Tobacco Use
    • Mental Health
    • Immunizations

     

    This information can be used to identify solutions so you can live a healthier, happier, and more productive life.
